Appointments
New appointments are always welcome and we are currently accepting new patients into our practice. If you would like to make an appointment, please contact our office by phone at (408) 252-3292.
Financial
We will be more than happy to submit all insurance forms for you to facilitate insurance benefit payment. We will do everything we can to help you afford the treatment you need and want. Forms of payment accepted by our office include check, cash, Visa Card, and Master Card. We are contracted health care providers for a number of health care plans.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. A fee may be charged if sufficient notice is not given. We ask for this advance notice so that we can offer this appointment to another patient in need of care.
